Dedicated Joyce Highland experiences turmoil when she tries to help a troublesome patient this week.
On her rounds, Joyce (Renee Bailey) is making visits to curmudgeonly pensioner Alf Cottered (David Troughton), who's recently had prostate surgery and lives in a squalid tenement flat. But Joyce battles to keep her cool when Alf makes racist digs.

'Alf's an example of the ignorance Joyce faces daily,' says Renee. 'There's friction and Joyce isn't afraid of confrontation, but she'll never compromise her professionalism and she wants to help because Alf needs her.'
Meanwhile, Joyce also meets Alf's neighbour, impoverished widowed mum Nerys (Georgia Landers), who's struggling to raise her children alone. But caring Joyce is torn over how to assist the family.
